Nebraska Man to Serve 3-Year Sentence for Medicare and Medicaid Fraud

Nebraska resident Mark L. Koehler has been found guilty of making false claims against Medicare and the state Medicaid program, and he has been sentenced to serving three years in a federal prison. This prison term will be followed by five years of court-supervised release. Mr. Koehler’s healthcare fraud cost the programs $1.4 million.

Mr. Koehler was formerly the business manager for Heartland Physical Therapy in Norfolk, Nebraska. The Omaha World-Herald reported that he has been ordered to pay restitution of over $1.8 million in restitution, which includes $500,000 to be paid to a bank.

Mr. Koehler’s fraudulent acts include billing Medicare and Medicaid over $1.3 million for services that were not performed. In addition, he used the false Medicare and Medicaid claims in reports that he used to fraudulently obtain cash advances of $500,000 from BankFirst of Norfolk.
